Ensure is offering a full rebate (up to $10) on your purchase of one multi-pack. To receive your rebate, follow these steps :
1. Buy one multi-pack of any Ensure product. Minimum purchase price is $6.97. Even if your purchase price is less after coupon, this should still work. There is no way to tell what product a coupon was used for on most receipts.
2. Print and complete the rebate form, send it along with your original receipt with product price circled and original UPC cut from the package to the following address :

Limit one request per household, per product. Purchases must be made by 12/31/09 and received by 01/13/10.

Could not find rebate form on health central website for ensure. Rebate
does not expire until the end of the year and just found the rebate
listing in a current magazine. What’s up!!!

Took me a while to find the magazine. It was in the Woman’s Day, November 3, 2009 issue on page 32 . It is listed under the six cash-back offers.
